Need a copy of your medical record?

Print or save the Release of Information Authorization form. This form is for requesting a copy of your information for yourself, transfering of records, or whatever you might need to receive quality healthcare. Please take the time to complete the form, if incomplete it will delay the release of your records. Need help? Instructions print with the release form or call 217-223-8400, ext. 6602

Once you have filled out the form, either mail it to Health Information Management, Correspondence, P.O. Box 7005, Quincy, IL 62305 or fax it to 217-223-6895.   By law, copies of records may take up to 30 days to receive, please call early if you need your records for an out-of-town appointment.

To check on the status of your request, please call 217-223-8400, ext 6613 or ext. 6614.

Please note there may be a fee for records requested. 

Midwest Medical Record Association (MMRA), is our copy service that will handle your request and is on site, Monday – Friday, 8:00 am to 4:30 pm. 

Location

Health Information Management Department (HIM) is located at Blessing Hospital 11th Street Campus, on the 1st floor down the hall from Patient Access, near elevator D. Our hours are 8 a.m. to 4:30 p.m. Monday through Friday.

HIPAA

The Health Insurance Portability and Accountability Act (HIPAA) of 1996 (P.L.104-191) [HIPAA] was enacted by the U.S. Congress in 1996.  The effective compliance date of the Privacy Rule was April 14, 2003. HIPAA establishes regulations for the use and disclosure of Protected Health Information (PHI). PHI is any information held by a covered entity which concerns health status, provision of health care, or payment for health care that can be linked to an individual.
